Non Accommodative ET

Let me tell you how we can manage non-accommodative esotropia (ET). Well, the approach in managing patient with non-accommodative ET may differ according to the case. Here are some useful guideline for you.

Presentation

ET same for N and D
No improvement with plus correction.
Normal AC/A ratio
Amblyopia is common
Onset: 2 years of age - even in late adulthood.
BSV is weak
Full duction

Differential diagnosis

Breakdown of an esophoria (most common)
6th nerve paresis
Myasthenia gravis
Tyroid eye disease.

Management

Conservative treatments such as vergence therapy, prism are usually not effective
Surgery is required: bilateral medial rectus recession

*it is not that easy to treat this kind of patient, after the strabismus surgery, patient might end up with diplopia. So, what should we do? Comment and share your opinion.

Kamu mgkin tak perasan, membaca @ study @ fokus pada aktivit-aktiviti dekat tanpa rehatkan mata boleh wat power mata korg naik. Tp ini untuk kes kanak-kanak la, untuk dewasa mungkin kurang berkesan.

Implikasi membaca tanpa henti ni leh sebabkan kamu pening terutama area dahi (temporal) dan kepala bhgn tepi (temporal). Ini sebab mata terlebih guna atau terlebih akomodasi. Jadi tak hairan la kalo korg pening time study lama2, bukan otak xle nak absorb, tapi mata kamu dah x leh nak terima guys!. Penat. Masalah ini akan lebih teruk untuk org yang mmg dah ada masalah untuk fokus dekat lama-lama sebelum ni. Bila baca lama2, huruf dalam buku tu jadi double, pening, cara baik nak hilangkan diorg pun tido. Akhirnya x study la malam tu.

Jadi apa yang perlu kamu buat? Baca post ini... pasal visual hygiene. Ambik mana yg anda bleh apply. Ingat study tu study jugak, tapi kalo asyik pening je mmg susuah ar nak masuk.

We just received a letter from UMP inviting us to hold an eye screening programme during Symposium in Biotechnology 2010 (Symbion 2010).

The details of the screening programme are as below:

Date/Day : 18th October 2010/Monday
Venue : Block W, UMP
Time : 8.00 am - 5.oo pm

Not merely eye screening, we will be having an exhibition as well.

The HOD has decided to make it compulsory to all final years students to involve into this programme. They will be grouped into two. The first group will go for the first session (8.00 am - 12.30 pm), while the other one will start from 12.30 pm until 5.00 pm (2nd session).

There are three vacant seats to be occupied by other batches (year 1 - year 3). Two seats for morning session and one seat for the evening session. For those who are interested to join this programme. Please contact Br. Azuwan latest by this thursday.

Visual Hygiene: What for?

Hi everyone!

Previously, we has discussed on how near work can induce myopia. Have a look at this again. Clinicians have long recognised that sustained periods of near work tend to induce myopia in susceptible individuals. Many researches suggest that accommodation plays a critical role in the progession of myopia.

Can we actually control the progression of myopia? Yes, sure!

There are few alternatives that practitioners or you can do beside using optical form of therapy. Try this, Visual Hygiene.

Visual hygiene is simply to say an activity that one person can do to relax his accommodation, so that the myopia is controlled.

Reading and writing activities at the distance from mid-knuckle to elbow, estimated by placing an elbow on the table surface and resting the chin in the palm of that hand

.Reading and writing on slant board at a 20 degree of angle.

Nonglare source of lighting and good posture. Please don't read while lying on bed!

Computer screen viewing at 16-30 inches (40-75cm)

Computer screen viewing angle 10-20 degree downward.

15 minutes break for each hour of computer use.

20/20 rule: Shift to distance fixation for >20 seconds every 20 minutes

Biosynthetic corneas restore vision to humans

A study made public this Wednesday has shown that biosynthetic corneas can and do restore eyesight in humans. Researchers from the Ottawa Hospital Research Institute (OHRI) and the University of Ottawa in Canada, along with Linköping University in Sweden, conducted a clinical trial using ten Swedish patients with advanced keratoconus or central corneal scarring. Each patient had the damaged corneal tissue in one eye surgically replaced with a biosynthetic cornea made from synthetically cross-linked recombinant human collagen. After two years, six of the patients’ vision had improved. After being fitted with contact lenses, their vision was comparable to that of someone who had received a real human cornea transplant.

"This study is important because it is the first to show that an artificially fabricated cornea can integrate with the human eye and stimulate regeneration," said Dr. May Griffith, who holds positions at OHRI, the University of Ottawa and Linköping. "With further research, this approach could help restore sight to millions of people who are waiting for a donated human cornea for transplantation."

Over a decade ago, Dr. Griffith started developing biosynthetic corneas in Ottawa, using laboratory-produced human collagen molded into the shape of a cornea – collagen is what “real” corneas are made from, too. After extensive testing, she began her collaboration with Dr. Per Fagerholm, an eye surgeon at Linköping, to provide the first-in-human experience with biosynthetic cornea implantation.

The patients in the trial didn’t experience any rejection reaction or require long-term immune suppression, which are complications that sometimes occur when human tissue is transplanted. During the two-year follow-up period, the researchers observed nerves and cells from the patients’ own corneal material growing into the implants, ultimately resulting in normal-looking “regenerated” corneas. The biosynthetic corneas also became sensitive to touch, and stimulated tear production to keep the eye oxygenated.

"We are very encouraged by these results and by the great potential of biosynthetic corneas," said Dr. Fagerholm. "Further biomaterial enhancements and modifications to the surgical technique are ongoing, and new studies are being planned that will extend the use of the biosynthetic cornea to a wider range of sight-threatening conditions requiring transplantation."

These are some useful informations that we can share from the 3rd ASEAN Optometric Conference 2010

The progression of refractive error in one person is contributed by two major factors: Nature & Nuture.
It is said that minus lense leads eye to grow longer while plus lense will reduce the growth. Reduced growth?
Peripheral Defocus cause myopia to progress in children.

Contribution of Myopia Progression

Sustained near vison tasks - When we do intense near tasks without break, we are actually causing myopic far point to shift.
Frequent change in accommodation - Induced contraction of ciliary muscle leads to an elongation of the axial lenght of the eye ball.
Print size - Smaller the print size, more accommodation is needed to keep it clear on the retina.

So, what are the possible mechanism that we can do stop reduce the myopic progression? They said - Outdoor activities!

In a day light, we accommodate lesser as compared to dark room.
Our pupils are constricted in light, hence we can have greater depth of focus.

Dopamine and Vitamin D in sunlight seem to inhibit the eye growth in certain circumstances.

Well, really don't know what suitable topic to be posted here. Emm...let us discuss on this topic. Latent hyperope.

Millodot defines latent hyperope in Dictionary of Optometry and Visual Science as a portion of total hyperopia which is compensated for by the tonus of the ciliary muscle. It can be revealed wholly or partially by the use of a cycleplegic.

It is suspected when:

patients have an AA less then their expected age.

patients are esophoric at distance through the manifest plus refraction.

the results of the binocular balancing show considerably more plus than the monocular subjective refraction.

when static retinoscopy shows considerably more plus than the subjective refraction.

the patients' plus prescription does not reduce the patients' near symptoms, such as headache.

Doctor of Optometry is now offered at University of Melbourne, Australia as an option for you to become an Optometry Doctor (OD) . It is a four years study at Deparment of Optometry and Vision Sciences which will consist of a combination of on-campus trainings and clinical placements. The application for admission is opened to all starting this year. Meaning to say, they will start the first session on July 2011. Great!

Malaysia Optometry Day (MOD) 2010 will be held on this September 2010. All optometry students and staffs are warmly invited to join the launching ceremony at Taman Tasik Titiwangsa, Kuala Lumpur.

v Blinking helps to wash tears over our eyeballs. That keeps them clean and moist. Also, if something is about to hit our eye, we will blink automatically.

v The eyeball of a human weighs approximately 28 grams.

v The cornea is the only living tissue in the human body that does not contain any blood vessels.

MOSSE 2010

Malaysia Optometry Students Sports Event 2010 (MOSSE 2010) was successfully organised by our department. Thanks to all committees members, lecturers, staffs and participants for being very supportive. It is an arden hope that this programme will be organised again, on the next session by the other school of optometry.. Insya-Allah.

Sorry for the inconveniences that were occured throughout the one-day programme. The committees took only three weeks to plan for the event, starting from proposal writing until the event end. Actually, we wished to gather all schools at that time, but then, due to last minutes invitation, only IIUM, UKM and UiTM can make it. Thanks, guys.

We'll post some photos later.

Looking forward to see all of you again on 'Hari Optometri Semalaysia' this December!
